• Full name of the insured should be the same as in their government approved IDs.
• Neoassure shall not be held responsible for any issues arising due to discrepancies in above details and KYC documents submitted during the claim process.
• The insurance will be activated immediately upon receipt of the premium.
• The insured must provide the necessary documents asked for by Insurance Companies to claim for the Insurance Services.

Declaration

I declare that:

• I am over 18 years and less than 55 years of age.
• I agree to share my information including Personal Information with MetLife, its Group Companies, reinsurance companies both within and outside Nepal for the purpose of insurance.
• I authorize MetLife to collect my medical information from any doctor, hospital, clinic, or medical providers.
• I declare that the information provided is accurate and complete.
